Kokoro^™ Counseling Center follows client record confidentiality and storage guidelines according to the strictist interpretation of our governing bodies. f the patient is a minor, the patient’s Personal Health Information (PHI) shall be retained or a minimum of seven years from the date the service is terminated, or when the patient reaches 18 years of age. After these time frames, PHI can be destroyed upon patient request. Only Kokoro^™ Counseling Center employees have access to the health records of present and past client files and strictly on an as-needed basis. Kokoro^™ Counseling Center utilizes PHI secure and HIPAA compliant processes in our website and in-office systems. All client PHI is securely stored and remains accessible per client request.
